1. In your home or work environment (or other familiar place), describe a system that presents a hazard. Specifically,

A. Describe the mishap that could occur.

B. Next, describe the risk of such a mishap occurring. Is the mishap likely or unlikely? What indicators help prevent the mishap? Are there distractions or other factors that make the mishap more likely? Is the system more dangerous to particular individuals such as the very old, the very young, or the disabled, and why?

C. Finally, give your opinion on whether the system could and should be made safer. Consider the impact on the utility of the system and its cost weighed against the risk the system poses.

2. Describe a situation that made you revise your estimate of the risk involved. What was the situation? What was your initial perception of the risk? What made you change your mind? How did the change in the perceived danger affect your behavior?
